After a conference with Judge Herren, it has become clear that continued use of the DSO Affidavit was not necessary. This form was developed after the 2005 amendments to the Bankruptcy Code in an effort to ensure compliance with 11 USC 1325(a)(8). I have removed the form from the web site and its use will no longer be routinely requested from debtors with DSO obligations.
